Returning Student Instructions

Applying for the 5-Week Summer Ballet Program

NOTE: These instructions are for students who attended the 5-Week Summer Ballet Program or August Course since 2015, and are not current academic year CPYB students. 

STEP 1: Login to your Family Portal. For password assistance, click the “forgot my password” link.

STEP 2: Update your MY ACCOUNT tab

  • Confirm/update family address, phone and emergency contact information
  • Confirm/update payment information
    • The account number you enter will be used to charge your application fee, the non-refundable deposit and other program charges. You may update your payment information at any time.
  • Confirm/update billing address
  • Click SAVE in the top left-hand corner
  • You can change your user ID and password through the “My Account” tab at any time


STEP 3: Update your MY CONTACTS tab

  • Complete all fields in their entirety
    • The contact should be the parent/guardian
    • If the student is over 18, they should complete their information in both the My Contacts and My Student tab and label themself as “adult student”
  • IMPORTANT: Confirm that e-mail and phone numbers are current
  • Click SAVE in the top left-hand corner


STEP 4: Update your MY STUDENTS tab

  • Select student applying for 2018 5WSBP
  • Completion of all fields is REQUIRED
    • IMPORTANT: It is critical that student cell phone and e-mail information is entered
    • Be sure "Receive Text Message Notifications" is checked next to the student cell phone number
  • To add another student to your family record, click "Add new student" in the top left-hand corner
  • Click SAVE in the top left-hand corner


STEP 5: Go to EVENT REGISTRATION tab (do not go to ENROLL tab)

  • Click on EVENT CALENDAR
  • Select "5WSBP Application"
  • Select the student you wish to submit the application for
    • Each individual student must complete their own application form
       
  • Complete ALL questions as instructed on form
    • Question #1: Enter "returning summer student"
       
    • Question #4: List previous CPYB level(s) and year(s) attended. 
       
    • Question #6: REQUIRED PHOTOS
      Applicants must submit the following three photos to CPYB's Dropbox account:

      - Photo #1, Head shot: Close up, full face, with hair up
      - Photo #2, Croisé fourth in relevé
      - Photo #3, Attitude derriere effacé

      For photos, females should be in leotard, light-colored tights and en pointe if appropriate. Males shuld be in tights, white t-shirt and ballet shoes.

      Uploading photos to Dropbox:
      1. Be sure your photos are labeled in the following way: Student Last Name_Student First Name_Photo #
      2. Click HERE to access CPYB's Dropbox folder for photo uploads.
      3. Upload your files by choosing "Choose from computer" or "Choose from Dropbox" if you already have an account

      NOTE: Photos should be submitted within 48 hours of submitting electronic application. Your application will not be considered until all three photos are received.
       
  • Question #7: A family may request scholarship assistance if the application is received prior to 1/20/18 and section 7 is completed IN FULL. If scholarship assistance is desired, please select “YES” from the drop-down box. Questions 7A-7E must be completed IN FULL and electronic copies of 2016 federal tax forms (1040, 1040A, 1040EZ) must be uploaded to CPYB's Dropbox in order to be considered for scholarship. If the date of application is beyond 1/20/18, continue to the Agreements Section of the application.

    Uploading tax forms to Dropbox:
    1. Be sure your tax form is labeled in the following way: Student Last Name_Student First Name_2016 Tax Form
    2. Click HERE to access CPYB's Dropbox folder for tax form uploads.
    3. Upload your files by choosing "Choose from computer" or "Choose from Dropbox" if you already have an account

    NOTE: Tax forms should be submitted within 48 hours of submitting electronic application. Your scholarship application will not be considered until tax forms are received.

     
  • Complete the Agreement Section
    • Each applicant is required to read and agree to the Central Pennsylvania Youth Ballet’s policies, including the 2018 5-Week Summer Ballet Program Payment and Refund Policies, by checking the box at the end of each policy listed within the application form
       
  • Payment verification: confirm that your account information is correct
     
  • Click SUBMIT REGISTRATION
    • Following submission, a confirmation e-mail will be sent to the contact e-mail address
    • Your $85 non-refundable application fee will be charged within 48 hours of application submission
    • Applications are not considered complete until required photos and federal tax documentation (if applying for scholarship) are submitted


Students will be notified of their enrollment status within 5 business days. Once accepted, you must reserve your place in the program and choose a payment date. When you reserve your place, a $300 deposit will be charged via your account within 48 hours. Tuition will be charged on the payment date you select.
