New Student Enrollment Instructions

Enrolling in the 2017-2018 Academic Year

NOTE: These instructions are for students who are NEW to CPYB and did not attend any CPYB programs in 2017. Please read these instructions before completing the application form.

Primary and Pre-Professional divisions scholarship applications for the 2017-2018 Academic Year are no longer being accepted. Please contact us with any questions.

Please note that one of the following payment methods is required to register:

  • Credit card payment: requires the name as it appears on the card (this will also be the BILLING CONTACT NAME within the registration form), card type (Visa, MasterCard or Discover), card number, card expiration month, and card expiration year.
  • eCheck/bank draft payment: requires bank name, bank routing number (9-digit number), account name (name on bank statement and this will also be the BILLING CONTACT NAME within the registration form), account type (checking, savings), and account number.
     

STEP 1: Complete FAMILY INFORMATION

  • The "Family Last Name" can be different than the student's last name
  • Billing contact #1 should be the parent/guardian
  • If the student is over 18, they should complete their information in the contact fields
  • The first and last name must be the same name as on the payment account that will be used
  • Enter emergency contact information that is different than the contact listed above
     

STEP 2: Complete STUDENT #1 INFORMATION

  • Completion of all fields is REQUIRED
    • IMPORTANT: It is critical that student cell phone and e-mail information is entered
    • Click "Receive Text Message Notifications" to ensure your child will receive notifications in the event of an urgent communication need
  • Enter student ethnicity as one of the following values:
    • American Indian or Alaska Native, Asian, Black or African American, Native Hawaiian or other Pacific Islander, White
       

STEP 3: REGISTER IN A CPYB PROGRAM

  • Next to "Select Class #1" click the "SEARCH" button
  • Select “Academic Year 2017-18” from the “Session” drop-down menu located at the top of the page
  • Under "Which CPYB Program?"
    • Primary and Pre-Professional divisions:
      • Select the registration category based on location
      • Select the student(s) you wish to enroll
      • Click "Enroll"
    • Children's Division
      • Select the registration category based on location and class. Students can take class at one location only.
      • Select the student(s) you wish to enroll
      • Click "Enroll"

STEP 4: COMPLETE THE AGREEMENT SECTION

  • Each applicant is required to read and agree to the Central Pennsylvania Youth Ballet’s policies
  • Credit Card Verification
    • The account number you enter will be used to charge your application fee, the non-refundable deposit and other program charges. You may update your payment information at any time.
       

STEP 5: Click SUBMIT REGISTRATION

  • Following submission, a confirmation e-mail will be sent to the contact e-mail address
  • Your $35 non-refundable registration fee and $45 non-refundable security fee will be charged within 72 hours of application submission
     

Once you complete the application, you can manage your account through your Family Portal

IMPORTANT NOTE: All students registered for the upcoming Academic Year are required to submit CPYB's Health Form Package in its entirety. Log on to your Family Portal --> Click the EVENT REGISTRATION tab --> Click the EVENT CALENDAR -->  Select “Health Form Package” on September 1, 2017--> Review instructions within the form, complete, and submit. Please note: this Health Form Package will be valid through August 2018.
