New Student Registration

For students who are NEW to CPYB and did not attend any CPYB programs 2015-2018. 

Please read these instructions before completing your registration. Should you have any questions regarding this process, please contact us at 717.245.1190 or info@cpyb.org.

BEGIN REGISTRATION

STEP 1: Complete REFERRAL and FAMILY INFORMATION
  • The "Family Last Name" can be different than the student's last name
  • Billing contact #1 should be the parent/guardian
  • If the student is over 18, they should complete their information in the contact fields
  • The first and last name must be the same name as on the payment account that will be used
  • Enter emergency contact information that is different than the contact listed above
     
STEP 2: Complete STUDENT #1 INFORMATION
  • Completion of all fields is REQUIRED
    • If your student does not have a cell phone, please enter the cell phone number of the Billing Contact #1.
  • Enter student ethnicity as one of the following values:
    • American Indian or Alaska Native, Asian, Black or African American, Native Hawaiian or other Pacific Islander, White
       
STEP 3: REGISTER FOR A CPYB PROGRAM

  • Click SELECT CLASS*
  • For Primary and Pre-Professional Divisions, select the registration based on location
  • For Children's Division: 
    • Creative Movement I students should register for ONE class (max) based on location
    • Creative Movement II students can register for TWO classes (max) at the SAME location
    • Pre-Ballet students must register in TWO classes at either location, and may also register for tap class(es) in addition to the minimum requirement of two ballet classes
  • Upon completion of STEP 3: If you have more than one student enrolling, click "ADD ANOTHER STUDENT." You will be prompted to complete STEP 2 and 3 again for your second student.  

STEP 4: COMPLETE THE AGREEMENT SECTION
  • Each applicant is required to read and agree to the Central Pennsylvania Youth Ballet’s policies
  • Credit Card Verification
    • The account number you enter will be used to charge your application fee, the non-refundable deposit and other program charges. You may update your payment information at any time.
  • If under the age of 18, the parent/guardian must enter their full name as the financially responsible party. 
     
STEP 5: COMPLETE YOUR METHOD OF PAYMENT (Required for registration)
  • Credit card payment: requires the name as it appears on the card (this will also be the BILLING CONTACT NAME within the registration form), card type (Visa, MasterCard or Discover), card number, card expiration month, and card expiration year.
  • eCheck/bank draft payment: requires bank name, bank routing number (9-digit number), account name (name on bank statement and this will also be the BILLING CONTACT NAME within the registration form), account type (checking, savings), and account number.
 
STEP 6: SUBMIT REGISTRATION
  • Following submission, a confirmation e-mail will be sent to the contact e-mail address
  • Your $35 non-refundable registration fee and $45 non-refundable security fee will be charged within 72 hours of application submission
     

Once you complete the registration, you can manage your account through your Family Portal
